Central Buckhead Property Corp d/b/a the Animal House Pet Hotel & Spa (the “Hotel”) and the undersigned owner (the “Owner”) of the pet (the “Pet”) named below entered into this agreement regarding the Hotel that Owner has read the Rule, Regulations and Policies of the Hotel and agrees to same;
i. that Owner has read the Rule, Regulations and Policies of the Spa and agrees to same;
ii. that the Pet is up to date on all vaccinations and has not been exposed to any transmissible illness (rabies, distemper, kennel cough, etc) within 30 days prior to boarding and/or shown any signs of illness (vomiting, diarrhea, extreme lethargy, etc) within 5 days prior to boarding;
iii. that the Hotel is authorized to request and obtain copies of the Pet’s medical and vaccination records from the Pet’s veterinarian and is authorized to engage the services of a veterinarian and/or to administer medicines as needed in the sole discretion of the Hotel and be reimbursed by Owner for any costs related to such care;
iv. that the Pet flea treatments are up to date but should the Hotel determine in its sole discretion that a flea treatment/bath is necessary, the Hotel is authorized to administer same at the Owner’s expense;
v. that animals have unpredictable personalities which at times may lead to injury and that the Hotel is not responsible for any injures or damages to the Pet resulting therefrom except in cases of sole gross negligence or intentional acts by an employee/agent of the Hotel but that in no event shall the Hotel, its agents or employees, be liable to the Owner in an amount in excess of the current chattel value of the Pet;
vi. that the Owner agrees to pay all charges for special services requested, all veterinary costs for pet during the time said pet is in the care of the Hotel, and all fees incurred by Owner through Owner’s failure to comply with the Hotel’s Cancellation Policy;
vii. that the Owner releases, indemnities, agrees to hold Hotel, its employees and agents, harmless and covenants not to sue same from any and all claims of loss, liability, expenses, damages, costs (including attorney’s fees) arising in law or equity from or related to the boarding and/or care of the Pet by the Hotel or any services provided by the Hotel except such claims which may arise from the sole gross negligence or intentional acts of an agent or employee of the Hotel, including, but not limited to: (a) actions of or by the Pet or by other boarded pets (including the destruction of property, bites and/or transmission of disease); (b) any inaccuracies in any information provided to the Hotel regarding care instructions and/or regarding the Pet; and/or (c) any acts which are breach of this agreement and/or the Hotel’s rules, regulations and/or policies by the Owner; and,
viii. that the Pet shall not be released to any person other than the Owner but if the Owner wishes the Pet to be released to a person other than the Owner, the Owner must provide the Hotel (unless such person is identified below as the “Owner’s Agent”) with specific written permission to do so and a method of verifying such person’s authority to include personal picture identification.
viiii. that the Owner agrees and allows, without any compensation, that the Pet may be photographed and said photos used in advertisements for the Hotel.

RESERVATIONS
To ensure quality service, reservations can only be guaranteed with a one-night advance deposit at the time of booking. Deposits are fully refundable if cancellations are made no later than 5 days prior to arrival. During the holidays or peak seasons, we require a two-night deposit with a full refund if cancellations are made no later than ten (10) days prior to arrival. Animal House Pet Hotel and Spa proudly accepts cash, Visa, MasterCard, Discover and American Express.
We request 24-hour notification for cancellation of day camp, and day spa, so that we may reschedule without charging for missed appointments.
We require a two-night minimum stay for the long-weekends of: Easter, Memorial Day, Labor Day, Independence Day, Thanksgiving Day, Christmas, and New years day, .
Accommodations are available for those who would like their family’s pets (of the same species) to stay together. There is a 20% discount (of the nightly suite rate) for each additional pet.
HOURS
Animal House Pet Hotel and Spa cares for guests, 365 day per year. We are open from 7 a.m. – 7 p.m. (Monday – Friday), 8 a.m. – 5 p.m. (Saturday), and 11 p.m. – 4 p.m. (Sunday). We are closed to the public on all major holidays. We shall make every attempt to accommodate special requests for early check-ins. There is always a full charge for the first day of stay, regardless of check-in time.
Check-out time is 12 noon. Guests leaving after 12 noon will be charged an additional $20 fee unless scheduled for day camp or grooming the day of check-out. We regret that we cannot accommodate checkouts during non-business hours.
HEALTH CARE
The health and safety of each guest is our number one priority, therefore, every guest must have a Hotel Guest Journal form completed prior to his/her stay. This profile allows us to get to know your pet on a personal level, understand his/her handling preferences and to utilize the medical records, personality profiles and special needs information for the health, safety and well-being of your pet.
All guests must be in good health and must not have had or been exposed to any contagious or communicable illnesses within a 30-day period prior to check-in. Animal House Pet Hotel and Spa advises against boarding guests that have a terminal illness and are in the late stages of that illness.
Each guest receives a pre-entry evaluation prior to admittance. Guests showing signs of vomiting, coughing, gagging, sneezing, or diarrhea will not be admitted. Guests showing signs of fleas or ticks will be treated at owner’s expense.
Guests must possess a temperament that allows staff members to be able to properly care for them. Allowances may be made for specific temperament issues, but guests cannot pose a danger to staff members.
We discourage bringing toys or bedding for your pet since we offer a large selection of toys and comfortable, clean bedding which meet our safety and sanitary standards. In order to minimize separation anxiety, it is acceptable to provide an old shirt containing familiar scents. However, in such event, we cannot be responsible for any lost or damaged items.
In the event your pet should become ill or injured, every effort will be made to contact you, your veterinarian of record, and/or your designated emergency contact for instructions. If a life threatening illness/injury occurs, Animal House Pet Hotel and Spa, in its sole discretion, may engage the services of a veterinarian or a small animal emergency hospital for evaluation, regardless of Owner’s instructions on the pet’s profile form. For any health concerns that may arise, our pet care professionals maintain a detailed log - which will be provided to you and your veterinarian.

VACCINATION REQUIREMENTS
All guests must meet our vaccinations standards. Animal House Pet Hotel and Spa requires a Vaccination & Health Certificate from a veterinarian before admittance can be allowed.
Puppies must be at least 9 weeks of age and have completed their first two series of vaccinations, including the Bordatella vaccine and a fecal exam. Puppies over 18 weeks of age must have their full series of vaccinations, including the rabies vaccine.
Guests requiring vaccination updates must have the vaccination administered no less than seven days before any grooming service or overnight stay is scheduled.

FOOD & MEDICATIONS
Our house cuisine is an All natural grade A ingredient, holistic, hypoallergenic, human grade, maintenance formula dog food made by Natura. House cuisine is available for $3 per day. You may choose to provide your own food (at no additional charge), as long as each meal’s serving size is prepared in a separate ziplock bag with your pet’s name on it for the entire length of the stay.
Animal House Pet Hotel and Spa will administer oral and topical medications at $3 per day. Guests requiring specialized care (injections, splint/bandage changes, suture removals, etc.) are handled on a case-by-case basis, typically at $5 per procedure. All medications shall be provided by owner, clearly labeled and must include written instructions with guest's name, type of medication, dosage, and schedule.
GENERAL
All guests entering and exiting Animal House Pet Hotel and Spa must be on a leash or in a carrier until directed. Animal House Pet Hotel and Spa provides customized identification collars upon arrival, therefore, all carriers, leashes, collars and harnesses will be returned to owner upon check-in.
Animal House Pet Hotel and Spa reserves the right to refuse admittance to any pet guest for any reason, at any time, who lacks proof of vaccinations, displays signs of untreated or potentially contagious conditions, demonstrates aggressive behavior, or who fails our standard health and temperament policies.
Our prices, policies, services and hours are not negotiable and are subject to availability and change without notice. Upon admission for services, your signature of contract acknowledges your awareness and acceptance of our policies.
